Item Left ventricular outflow tract thrombus in a child with dilated cardiomyopathy: An atypical location(Journal of cardiovascular echography, 2017) Abdulkadir, Mohammed Baba; Ibrahim, O.R.; Afolayan, F.M.; Adedoyin, O.T.Dilated cardiomyopathy (DCM) may be associated with formation of intracardiac thrombi which may embolize and result in life‑threatening complications. We present a 19‑month‑old female child with DCM who presented with a right hemispheric cerebrovascular accident. Urgent echocardiography revealed poor left ventricular systolic function and a thrombus attached to the interventricular septum along the left ventricular outflow tract. There was resolution of the thrombus following the use of warfarin.Item Mucocutaneous disorders of Paediatric HIV in South-West Nigeria: Surrogates for immunologic and virologic indices(South African Medical Association, 2016) Katibi, O.S.; Ogunbiyi, A.O.; Oladokun, R.E.; Ernest, S.K.; Osinusi, K.; Brown, B.J.; Adedoyin, O.T.; Ojuawo, A.I.Objectives: Nigeria has the world’s highest burden of pediatric HIV. In the face of paucity of monitoring tests in Nigeria, we studied the spectrum of pediatric mucocutaneous manifestations and evaluated their clinical utility as surrogate markers for immunodeficiency and plasma viral load levels. Methods: Cross-sectional study comparing mucocutaneous manifestations in 155 HIV-positive children aged 12 weeks to 14 years with 155 HIV-negative children. Relationships between mucocutaneous man ifestations in HIV-infected patients and their immunologic and virologic indices were analyzed. Results: Mucocutaneous lesions were seen in 53.5% of HIV-infected children compared with 18.1% of the controls. Prevalence of lesions increased with worsening levels of immunodeficiency and increasing viral loads (P < .01). Oral candidiasis, angular stomatitis, and fluffy hair were associated with more severe degrees of immunodeficiency. Conclusion: Mucocutaneous disorders are common in HIV-infected children. Oral candidiasis and nutritional dermatoses can be used as surrogates for advanced or severe immunodeficiency.Item Open Heart Surgery in Ilorin: Case Report and Experience with the First Two Cases(College of Health Sciences, University of Ilorin, Nigeria, 2017) Adeoye, P.O.; Abdulkadir, M.B.; Kolo, P.M.; Ige, O.A.; Afolabi, J.K.; Ogunmodede, James Ayodele; Olawumi, H.O.; Adeboye, M.A.N.; Adedoyin, O.T.Open heart surgery is uncommonly done in Nigeria and restricted to few centres despite the burden of congenital heart disease. We present our first experience with open heart surgery at the University of Ilorin Teaching Hospital. A 15 years old female adolescent presented with a 2 years history of easy fatigability, dyspnoea on exertion and chest pain. Echocardiography revealed a large ostium secundum atrial septal defect. She had patch closure of the atrial septal defect. The second case was a 7 years old female child with stunted growth. Echocardiography confirmed an ostium secundum atrial septal defect and severe pulmonary valvular stenosis. She had patch closure of the atrial septal defect and pulmonary valvulotomy. Both patients are being followed up and doing well. The hospital has achieved a major milestone by performing its first open heart surgeries. A coordinated team approach yields good results in the development of capacity and facilities for performing open heart surgery in Nigeria.Item Oscillometric blood pressure profile and anthropometric indices among healthy school children in Ilorin, North-central Nigeria(Saudi Journal for Health Sciences, 2018) Ibrahim, O.R.; Adedoyin, O.T.; Ojuawo, A; Afolabi, Joseph Kolawole; Mokuolu, O.A.; Abdulkadir, Mohammed BabaContext: Oscillometric devices are preferred method for measuring blood pressure (BP) among children. Aims: This study measured BP among school‑age children using a validated oscillometric device (Omron 705 IT®) and correlated the findings with the anthropometric parameters, with a view to determine the predictors of BP. Settings and Design: This was a cross‑sectional, descriptive study. A multistage stratified random sampling technique was used in the selection of pupils from primary schools in Ilorin, Nigeria. Subjects and Methods: Two serial BPs were measured used using Omron 705 IT® with appropriate cuffs using “the fourth report” guideline and standard methods were used for measurement of anthropometrics. Statistical Analysis Used: Data were analyzed using SPSS version 20. Results: A total of 1745 primary school‑aged children comprising of 873 males and 872 females were recruited. Anthropometric parameters were comparable between males and females. Mean systolic and diastolic BPs were 103.8 ± 11.0 mmHg and 61.3 ± 8.4 mmHg, respectively. Mean systolic BP was lower in males compared with females (102.9 ± 10.6 mmHg vs. 104.7 ± 11.3 mmHg, respectively, P = 0.001). Mean diastolic BP in males was lower compared with females (60.7 ± 8.3 mmHg vs. 61.8 ± 8.5 mmHg, P = 0.009). Most of the anthropometric parameters correlated with the BPs. Independent predictor of BP was weight, height, and body mass index (BMI), P < 0.001. Conclusions: There was a weak correlation between the oscillometric BPs and anthropometric parameters with weight, height, and BMI been the independent predictors of BP.
